For us, Appropriate Care is not a theoretical framework from the Integrated Care Agreement (IZA), but a daily practice. It is our mission to make the transition from 'institution to home' not only possible, but also medically responsible and humane.

However, for complex cases, a weekly visit from community nursing is often insufficient. Through close collaboration with general practitioners and specialists, we ensure that the treatment plan is strictly and expertly implemented at home. 4. Focus on Health: Prevention through Presence Through 24/7 presence, we transform care from reactive to proactive. We identify complications before they become crisis situations. This prevents unnecessary emergency admissions and relieves the already overburdened acute care chain.
