For us, Appropriate Care is not a theoretical framework from the Integrated Care Agreement (IZA), but a daily practice. It is our mission to make the transition from 'institution to home' not only possible, but also medically responsible and humane.

The four principles of appropriate care at Happyhomecare

1. Value-driven: Quality of life as a compass. Care is only appropriate when it contributes to what is truly important to the patient. By facilitating care in their own living environment, we restore autonomy. Our live-in caregivers look beyond the medical record; they support the person behind the patient.

 

2. The Right Care in the Right Place: Home as a safe base. The most suitable place for recovery and aging is almost always at home. However, for complex cases, a weekly visit from community nursing is often insufficient. We offer the necessary proximity and coordination needed to immediately relieve the pressure on hospital beds and nursing homes.

 

3. Shared Decision-Making: The triangle between doctor, patient, and care provider. We act as the eyes and ears of the treating physician. Through close collaboration with general practitioners and specialists, we ensure that the treatment plan is strictly and expertly implemented at home. Appropriate care means that the patient remains in control, supported by a professional safety net.

 

4. Focus on Health: Prevention through Presence Through 24/7 presence, we transform care from reactive to proactive. We identify complications before they become crisis situations. This prevents unnecessary emergency admissions and relieves the already overburdened acute care chain.
